I.                   POSITION DESCRIPTION

 

Work Unit:    Park City-County Health Department

Supervisor:    Health Department Director

 

Current Classification: Pay Grade:  16

Wage: Pay Range $28.17 - $34.83        

Title: Community Health Worker

Non-Exempt, Permanent Part Time (up to .75 FTE or 30 hours a week)

Grant funded position through November 2027, possible extension based on available funding

 

II.                   ASSIGNED DUTIES & TASKS:

 

Position overview

This dynamic role adapts daily to meet community needs and requires clinical skills and flexibility. The position leads initiatives such as home visits, immunization clinics, community outreach, education, and disease investigations to promote public health in Park County. The position supports efforts to control communicable diseases by identifying sources, reaching at-risk groups, and educating patients on disease prevention and etiology—all while maintaining the highest standards of confidentiality.

All duties below are essential functions unless otherwise indicated. Duties of the position are not all inclusive and are subject to change.

 

PUBLIC HEALTH RESPONSIBILITIES:

 

MATERNAL AND CHILD HEALTH PROGRAM

  • Conduct voluntary home visits for families with newborns to assess health issues and home environment and provide education and referral information as needed.
  • Make referrals to other community resources agencies and specialists when appropriate.
  • Work with community partners on outreach and education around maternal and child health issues.
  • Assist with the Maternal and Child Health Program which includes quarterly reporting, conference calls and trainings.

 

DISEASE INVESTIGATIONS

  • Interview individuals to identify potential disease transmission sources and promote compliance with public health laws.
  • Advocate for patients by educating them on available medical and social services and facilitating access.
  • Conduct case investigations, document histories, and trace infection sources.
  • Support immunization activities under the Public Health Emergency Preparedness grant.
  • Respond to disease-related questions from the public and stakeholders.
  • Stay informed on local, national, and global disease trends and outbreaks.
  • Prepare contact reports and tracing documentation.
  • Develop and distribute outreach materials that address service disparities.
  • Assist in planning to improve access for at-risk populations.
  • Participate in state-led meetings, trainings, and information sessions.

 

IMMUNIZATION

  • Support seasonal vaccine clinics for the community and local businesses.
  • Assist with immunization clinics (e.g., influenza, HPV, Tdap, meningococcal) at schools and daycare and group home facilities.
  • Assist with school-based vision and hearing screenings.
  • Review immunization records for licensed daycare providers.
  • Collaborate with community partners on immunization education and outreach.
  • Assists with immunization grant duties, including reporting and trainings.

 

OTHER DUTIES AS ASSIGNED

Performs other duties as assigned by Health Director.  This job may include special projects and a variety of other functions as needed


III.             KNOWLEDGE AND SKILLS

This position requires clinical expertise (RN, LPN, NP, or PA), particularly in infant and maternal assessments and immunizations. Familiarity with—or the ability to learn—local and state public health resources is essential. Candidates must demonstrate strong communication, public speaking, collaboration, and independent work skills, while adhering to HIPAA and other medical/legal guidelines. Experience in community health engagement is valued. The role demands adaptability, cultural competence, and strong organizational abilities.

Technical proficiency with computers, word processing, data entry, and relevant software is required. Must be able to operate standard office equipment and drive defensively in all weather conditions.

KNOWLEDGE

  •  Maintain strict patient confidentiality during interviews.
  •  Understand medical terminology and key concepts related to infectious diseases and transmission.
  •  Stay current on communicable disease symptoms, transmission, treatment, and impact.
  •  Apply standardized protocols for investigating and intervention with positive cases.
  •  Demonstrate knowledge of modern disease counseling practices.
  •  Accurately complete medical documentation, record keeping, and business correspondence.
  •  Follow universal precautions and infection control procedures.

SKILLS

  •  Build and maintain trust with patients and contacts using strong interpersonal and interviewing skills.
  •  Exercise sound judgment and involve appropriate resources when responding to inquiries or releasing information.
  •  Monitor communicable diseases throughout Park County.
  •  Collaborate with Park County DES and PHEP on emergency protocols.
  •  Deliver excellent customer service with strong communication skills.
  •  Work accurately in a fast-paced, detail-oriented environment.
  •  Clearly explain information and use collaborative problem-solving to ensure legal compliance.
  •  Prioritize and allocate resources effectively in all situations.
  •  Prepare clear, accurate, and publicly accessible documentation.

Education and Experience

Job requires being licensed as a Registered Nurse, Licensed Practical Nurse, Physician’s Assistant or Nurse Practitioner. Experience (2 – 4 years) working in a clinical setting is preferred.  Experience working with the general public and people of varying backgrounds is needed. Experience with medical billing will be helpful. Experience working in the public health is preferred.

 

 Certificates, Licenses, Registrations

Montana State Board of Nursing License (RN, LPN, NP) – Active   Or Montana Boards of Medical Examiners License (PA)—Active

FEMA IS-700, IS-800, ICS-100 online courses within 6 months of employment and ICS-200, ICS-300 courses within one year of employment as mandated by the state.

Valid Driver’s license. 

IV.                 ACCOUNTABILITY

This role requires a strategic and independent professional to guide complex decision-making aligned with public health objectives. Reporting to the Health Department Director, the position ensures effective performance of essential duties within established policies, plans, and budgets. The position involves leading high-priority projects that often fall outside standard procedures; thus, requiring initiative, integrity, and sound judgment to accomplish projects. The ideal candidate will demonstrate leadership in navigating new and challenging situations while maintaining accountability and achieving meaningful results.

V.                   CONFIDENTIALITY

This position requires strict adherence to confidentiality when handling non-public, personally identifiable information, in accordance with federal and state law and agency policy. The employee agrees to respect and safeguard such information at all times. If confidential data is accessed unintentionally, the employee must promptly report the incident to a supervisor. Unauthorized use or disclosure is prohibited and may result in disciplinary action, up to and including termination. VI.                  INDEPENDENCE OF ACTION

This position carries out assignments independently, and uses judgment and initiative
to solve problems.  This position may operate with appreciable latitude for independent action and decisions commensurate with demonstrated ability. Work objectives and priorities are defined along with the supervision of the Health Director, but leeway is granted for the exercise of independent judgment and initiative.

 

VII.                 PERSONAL CONTACTS:

This position has contacts with the public, county and city personnel, elected officials, department heads, State, Federal and City Officials, Law Enforcement Agencies and various community organizations. The person in this position interprets, clarifies or explains departmental information in order to be responsive to questions and inquiries.

VIII.                WORK ENVIRONMENT/PHYSICAL DEMANDS

Work conducted in office and field environments. Due to the need for all Health Department personnel to respond to public health emergencies, the employee must be assessed for their ability to meet the physical demands of performing the following activities:

Physical Demands:

  • Must be fit-tested and able to wear a NIOSH N95 mask.
  • Physically able to sit, stand, walk varied terrain, climb stairs, bend, kneel, reach, grasp, and lift up to 30 pounds.
  • Capable of exerting up to 25 pounds of force occasionally, and minimal force regularly to move objects.
  • Sufficient hearing and verbal communication skills for in-person and phone interactions.
  • Visual ability to read documents, use a computer, and drive safely.

Work environment:

  • Work is conducted in an office setting and various community sites.
  • Fieldwork may involve exposure to inclement weather, hazardous conditions, travel risks, and potential contact with communicable diseases, toxins, and animals.
  • Interactions may include individuals displaying challenging or hostile behavior.
  • Use of personal protective equipment (PPE) may be required.
